A Secretaria de Estado de Saúde de Minas Gerais (SES-MG) realiza ações de conscientização até esta quinta-feira (8/2), na Semana Nacional de Prevenção da Gravidez na Adolescência, campanha de caráter educativo e preventivo em que são abordados desde a sensibilização da temática, entre o público adolescente e seus cuidadores, aos métodos contraceptivos disponíveis atualmente.
A campanha é direcionada a profissionais de saúde e interessados no tema, já que segundo a Organização Mundial de Saúde (OMS), a gestação na adolescência é uma condição que eleva a prevalência de complicações maternas, fetais e neonatais, além de agravar problemas socioeconômicos existentes. As complicações e gravidade da gestação correlacionam-se, principalmente, à idade. Tornar-se mãe durante a adolescência pode apresentar certas dificuldades e perigos porque a gestação neste período tem mais chances de complicações, tanto para o bebê quanto para a mãe.
De acordo com Lírica Mattos, diretora de Gestão da Integralidade do Cuidado da SES-MG, é importante entender que todo adolescente tem direito ao atendimento e acolhimento correto no Sistema Único de Saúde (SUS). Ela explica que pais, educadores, profissionais de saúde e toda a sociedade têm um papel fundamental na educação desses jovens.
“O primeiro passo para a prevenção da gravidez na adolescência é a informação qualificada. O SUS oferece métodos contraceptivos como pílula combinada, anticoncepcional injetável, dispositivo intrauterino (DIU), assim como preservativos, tanto femininos quanto masculinos, porque a responsabilidade da prevenção é compartilhada”, afirma Lírica Mattos, que acrescenta que as gestantes devem procurar os centros de saúde mais próximos de suas casas. “Lá, será ofertado acolhimento, planejamento reprodutivo e sexual, captação precoce de gestantes para a realização de pré-natal adequado, cuidado no parto, puerpério e cuidado com o bebê”, complementa.
Riscos
Em 2020, 14% dos nascidos vivos no Brasil foram de mães adolescentes e em Minas Gerais, entre 2019 e 2022, cerca de 10% dos nascidos vivos são filhos de mães na faixa etária de 10 a 19 anos.
De acordo com o Sistema de Informação sobre Nascidos Vivos (Sinasc), a Macrorregião de Saúde Centro registrou o maior número de gestações em adolescentes de todo o estado, com mais de 28 mil bebês nascidos entre 2019 e 2022, aproximadamente 25% dos casos em Minas Gerais. A Macrorregião de Saúde Norte está em segundo lugar nesse ranking, com 11.613 bebês.
A gestação na adolescência aumenta as chances de prematuridade e complicações durante o parto, como pré-eclâmpsia, desproporção pélvica-fetal, entre outras. A realização do pré-natal de maneira adequada impacta diretamente nesse desfecho. Segundo dados disponíveis no Sinasc, no ano de 2023, foram 20.961 nascidos vivos de mães adolescentes, sendo 758 de mães com idade entre 10 e 14 anos e 20.203 bebês de mães com idade entre 15 e 19 anos. Destas, 322 declararam não ter feito nenhuma consulta pré-natal.
Lírica Mattos ressalta que esse acompanhamento é uma das principais formas de prevenir a mortalidade materno-infantil e de garantir o acompanhamento adequado do desenvolvimento do bebê. “O pré-natal é um dos principais pontos de assistência para a saúde durante a gestação, deve ser iniciado no primeiro trimestre e garantido o número mínimo de seis consultas. Assim, aumenta-se a chance de desfechos favoráveis tanto para a adolescente quanto para o bebê”, explica.
A diretora de Gestão da Integralidade do Cuidado da SES-MG reforça que o planejamento reprodutivo também deve ser abordado durante a gestação, para que as mães recebam orientações e tirem dúvidas sobre os métodos contraceptivos disponíveis, de modo a prevenir futuras gestações ainda durante a adolescência.
“Caso o parceiro da adolescente esteja presente, é importante que ele seja implicado no processo de planejamento reprodutivo”, aponta Lírica, que também salienta que a gravidez em adolescentes de 10 a 14 anos é considerada estupro de vulnerável, dentro da legislação vigente, portanto toda oferta de cuidado é pautada no que preconizam as leis brasileiras. “A gravidez pode trazer complicações tanto para o bebê quanto para a mãe, então é muito importante trabalhar na perspectiva de prevenção. Outro ponto que temos trabalhado na SES-MG são as ações de enfrentamento, inclusive no Plano de Enfrentamento à Mortalidade Materna e Infantil, porque essa faixa etária é a que apresenta mais risco de mortalidade materna e prematuridade do bebê”, destaca Lírica Mattos.
Informação é a melhor forma de prevenção
Os desafios enfrentados pelas mães adolescentes, suas famílias e companheiros são múltiplos e, por isso, a necessidade de acolhimento, acompanhamento pré-natal e medidas de promoção e prevenção de saúde destas jovens. É necessário que o diagnóstico, manejo clínico precoce da gravidez, condução da gestação, acompanhamento no puerpério e o estímulo à amamentação, além do controle nutricional sejam realizados por equipe multidisciplinar para garantir o bem-estar físico, psicológico e social dessas mães adolescentes, dos parceiros, filhos e familiares.
A gerente financeira Camila Lares Pereira tinha 15 anos e cursava o segundo ano do ensino médio quando descobriu a gravidez. O pai da criança também era adolescente e tinha 17 anos. Ela relata que a gravidez foi um período complicado, que mudou sua vida por completo, e considera a orientação correta e a educação sexual essenciais.
“Eu era uma criança, precisei cuidar de outra criança e, para viver a maternidade, não pude vivenciar aquilo que é próprio dos jovens. Faltou orientação, tanto da parte da escola como das pessoas que me acompanhavam com relação ao uso de preservativo ou de medicamentos”, lembra.
Ela conta ainda que precisou superar inúmeras adversidades na época, desde a relação estremecida com o pai, retomada quando a barriga cresceu e o bebê já se mexia, até problemas fisiológicos que a gravidez acarretou. “O parto foi uma experiência única e me assustei muito. Depois, vieram as dificuldades da amamentação e de entender que tinha uma criança que dependia de mim. Pensei em largar os estudos, mas meus pais e minha família me incentivaram a continuar e esse apoio foi essencial. Além da família, recebi muito apoio da família do pai da minha filha, que fizeram tudo que estava ao alcance deles”, ressalta Camila Pereira.
Camila Pereira destaca que mantém um diálogo aberto com sua filha, Sabrina Lares Tonani, atualmente com 16 anos, para alertá-la da importância da prevenção de uma gravidez precoce. “Conversamos abertamente e claramente sobre métodos de prevenção e cuidados. A Sabrina é muito consciente, até porque eu conto a minha história para ela e falo sempre que, para ter relação, ela precisa se cuidar para que evite passar pelo que eu passei”.
